Since the late 1970s, in many countries in the global South, attempts to improve health and social welfare provision have been undermined by debt and structural adjustment policies dictated by the ‘realities’ of global economic competition.
The fact that it is being used to describe the interaction of three analytically distinct trends (each with its counter-trend) or, at least, to characterize their combined impact, suggests that the concept may obscure as much as it discloses about recent changes. Some conceptual clarity can be introduced by distinguishing between state- and governance-centred approaches. State-centred approaches tend to adopt the ideal-typical sovereign national state as their reference point and examine the European Union in one of two ways.
